Mix up of medication
There are many instances that patients have sued doctors and hospitals for medical malpractice cases in Toronto. One case that I believe I may have read about was how a doctor was giving babies the wrong medicine accidentally as the names of the medicine and look of the medication looked similar on the shelf so it was getting the doctors mixed up.
